What is Varicocele?

A varicocele occurs when veins inside your scrotum i.e. the pouch of skin that holds testicles, becomes enlarged.  The enlargement of veins in scrotum is due to malfunctioning of valves in veins, just like varicose veins in legs.

The enlargement of veins causes accumulation of blood in varicose testicular veins, which may affect sperm production and may cause male infertility.  The varicose testicular veins in the scrotum i.e. Pampiniform Plexus are found in the spermatic cord that holds the vas deferens and testicular artery that supplies blood to the testicles.

The main role of the pampiniform plexus is to cool the arterial blood before it reaches the sperm through heat exchange mechanism. It is necessary as creation of sperm is most efficient at around 34.5 degrees Celsius i.e. 94.1 degrees Fahrenheit.

Generally, temperature of blood is higher in body than in pampiniform plexus i.e. testicular veins; accumulation of blood in them affects the formation and quality of sperms which may be the possible cause of male infertility.

Varicoceles can also affect the normal development of testicles and may cause them to shrink.  Vericocele surgery, also known as Varicocelectomy is performed to remove those enlarged veins and restore proper blood flow to male reproductive organs and treat male infertility.

How varicocele surgery is effective in treating male infertility?

The aim of Varicocele surgery is to seal off the affected i.e. enlarged testicular vein and redirect the blood flow into other normal veins. Varicocele surgery increases chances of improving your fertility and reducing other possible complications due to the blocked blood flow into your reproductive organs.

The Varicocele surgery success rate range from 60 – 80% and hence considered highly effective in treating male infertility.

How a varicocele is surgically repaired?

Varicocele is surgically repaired in 4 different procedures such as:

  • Open surgery i.e. Varicocelectomy done through your groin (inguinal or subinguinal), abdomen or pelvis to clamp or tie off the abnormal veins.

Surgeons often use surgical microscope that enables the surgeon to see the treatment area better during surgery.

Now-a-days surgeons also make use of Doppler ultrasound which helps guide the procedure.

Open surgery using a microscope and subinguinal approach i.e.  Microsurgical subinguinal varicocelectomy is reported to achieve highest success rates when compared with other surgical methods.

  • Laparoscopic surgery – minimally invasive procedure performed using a laparoscope to view and repair varicocele.

Surgeon makes few small incisions to insert tiny instruments to cut varicose testicular veins that are blocking the blood flow.

The ends of the veins are sealed off using small clamps or by cauterizing them with heat. The tiny instruments and laparoscope are removed once the cut veins are sealed.

  • Percutaneous embolization or Varicocele embolization – Non-surgical and minimally invasive procedure performed by inserting a catheter into the body through the neck or groin.

Surgeon uses coils or chemicals to block the blood flow to the vein by scarring it. This is also a minimally invasive surgery and can be performed as an outpatient procedure.

The recovery time with this procedure is relatively short.

  • Radiologic Balloon Occlusion – is performed using a silicone balloon catheter that is passed under X-ray guidance to the varicose testicular vein.

The balloon is inflated and left in place to permanently block the engorged veins and repair the varicocele.

This method is not commonly used to repair varicocele.
